1. Free for all. 2. Even number teams 3. cluttered's not the right word, maybe something nice like busy? 4. c and above is good, though fewer is still fun 5. no flags or both. The experience is more pure that way.

Team Speak's the best third-party all around, but in-game integration is always nice to see. The only problem is it can take away from actual game development.
